If this has already been brought up, then I apologize for missing it, but are there any examples known of this happening with a copper planchet?

Do you think changing to the single squeeze process eliminated this problem?

 

I originally started thinking along those lines, but then tossed that idea as it is my understanding that we didn't start using the single squeeze on cents until the late '90's(97?) and also I would think that the hubbing process would only effect the dies and not the coins. I was just wondering if the focus should be on the die or the planchet.
