    The first print (back), second print (front) and third print (overprint) all went as planned. The completed 16-subject half sheet wasn’t sitting correctly in the stack for the final cutting into individual notes and that’s when the error occurred. And, replacement notes (star notes) are printed before they are needed, not afterwards.
