I can think of one, but it is so far fetched it is hardly believable. It would require a dollar and a quarter planchet to enter the coining chamber at the same time, the quarter planchet would have to be perfectly centered and then the coins struck. That would impress the quarter planchet into the dollar and create a raised rim all the way around the depression. The problem with this senario? First it is very unlikely to get both blanks in there at the same time. Second the coins at the mint are now struck on horizontally striking presses. Gravity works and would cause the quarter planchet to fall to one side if not completely out of the press. So the dies would have to just happen to come together and catch the quarter as it is falling out of the press as it just happens to be exactly centered in the dollar planchet. then after it was struck the quarter has to fall out of the dollar. Anyone really believe this is possible?

It's funny you mentioned this, because this was the only other scenario that popped into my head, but it is, shall we say, somewhat unlikely.
