A coin from the last days of the Ottoman Empire:

Turkey 20 Kurush (Piastres) AH 1327 (AD 1909) Regnal Year 9 (AD 1917)
    Silver, 37 mm, 23.85 gm
El Ghazi at right of Toughra (Sultan's signature)

The coin has a mark on the reverse right which looks like metal has filled in the area.

Is this a mint creation, error, or something else like jewelry work.

    :)

If I'm looking at the same place you are, I'm thinking that could be where something like a jewelry bezel or clip was soldered to the coin. Again, "could be ".
Although, it could be a cud, since it is touching the rim. Again, "could be".
