How does something like that get out of the Mint and past any type of quality control ?

Yes, like many coins/patterns/trials/errors in the past.

Security really isn't that hard. Ban cash inside the mint, make everyone go in and out of metal detectors. It is almost like there are others inside the mint helping this stuff get out besides line workers.

Agreed. It's rather hard to believe this is the result of just the low level imo.

Thanks. Was going to buy it, but he doesn't have one with a matching nut. How would one be expected not to loose it without the nut?
