That's gotta be one of the coolest mint errors I have ever seen

 

I'm trying to imagine the damage that caused to the machinery/dies.

 

How does something like that get out of the Mint and past any type of quality control ?

Yes, like many coins/patterns/trials/errors in the past.

Security really isn't that hard. Ban cash inside the mint, make everyone go in and out of metal detectors. It is almost like there are others inside the mint helping this stuff get out besides line workers.
