This is one type of PMD. What do you think happened?

    Seriously though probably something along those lines, or someone biting it more than a few times.

    This is an example of a TERRIBLE attempt by a :bucktooth: at hiding a damaged cheek. It is NOT what we usually refer to as tooling although a "tool" was used to dent the surface. A more sophisticated and deceptive repair is done with a very tiny rounded punch.

    PS In the hands of a really good "artisan," this coin can be made much more "presentable."
