This 1941 P looks like a struck through error

    Yes, a minor filled die - usually machinery grease

    I thought grease at first. But, from your experience, could grease cause that much of an indentation?

    Yes, sometimes the grease, or machinery 'stuff'
    gets hardened/compressed as coins are struck.

    If it was a lamination (what happened to the type size?) you'd
    see the ghost of the A, for example.

    I'll also say yes. I have found quite a few of the 2009P Formative Years with either the hand or book completely obliterated by being struck through compacted material.

    Grease filled Die. The indenting may be from a defective planchet as I see no signs of scraping. There are no rough edges from damage so I'm going with a minor defective planchet.
