Questions about Washington quarter errors

The obverse has a flake of metal lifting from the surface between B and E. The obverse is also struck slightly off center (rim wide at 10 o'clock and thin at 4 o'clock).</p><p>4. 1964-D with similar double reverse as the previous coin. It's also struck oddly.
