What does this say about the accuracy of their service?

Personally, I think they are fine for grading and authentication. But certainly lead much to be desired when it comes to attribution of even type. They just don't spend the time required to do that job and really shouldn't even offer the service at the current performance level. This is why you must buy the coin, not the label.

There are no grades assigned to coins in sample slabs.

That depends on what part of their service you're talking about. Sample slabs are put together by low level employees. The professional graders never even see them because the coins in samples are never graded.

The problem they have lies with those low level employees who print and insert the labels. And do they make mistakes ? Yup, they sure do, and they make them with graded coins as well. But if you find a mistake on a graded coin they'll fix it for you.

Will they fix a mislabeled sample slab ? I doubt it since the purpose is only to show you what the slab itself looks like. So it doesn't make any difference what the label says. So to me it doesn't say anything about the accuracy of their service.

If was going to worry about a TPG being accurate the only things that would concern me is their being accurate with grading and authentication. Their making a mistake on a printed label now and then wouldn't bother me in the least. That said, I would always trust the authentication done by NGC and PCGS. But I always question their grading, as should everyone.

That is a pretty old slab, at least 20 years old and maybe more. And yeah they did grade them back then but they quit doing it after that.

As for your question, in those slabs, yes. In those after that, no.

Some sample slabs have grades on them just to show where a grade goes on the slab, but they're not intended to be attributed to the coin.

Cameron Kiefer has a website about all this, and Heritage even samples it on their site.
