    I would guess an out-of-adjustment press.
    I assume that too much pressure was hard on dies/equipment so a less than perfectionist worker probably would err to the side of low pressure.

    That effect can be caused be several things mentioned on page one. We call that characteristic a "Weak Strike." Several of the answers were correct.
