Is this a Die Chip Error?

    Yes, they are the result of a weakness in the design of the die.

    It's a Utah state quarter, I don't know if you remember I had another Utah that had the same die chip in the B, I thought that was kind of funny.

    I found an oklahoma today with a die chip on the B and on the date!
