Sorry, I'm a little late to the party. I just saw this. Without doing an actual exam, I really can't give a firm 100% answer on what happened. But, here is what I feel is most likely. It looks to be either a second or third (wisdom tooth) upper molar based on the shape of the root and tooth itself. The only reason a tooth will just "fall" out is due to bone loss around the tooth. There are a couple reasons why this can happen. One is severe decay can cause an abscess at the end of the root that with enough time can cause the tooth to exfoliate on its own. Frankly, this is quite rare and unlikely. There could have been a hairline vertical root fracture on the tooth as well. That can cause this. Another reason is bone loss from the top of the tooth down (meaning not from the end of the root)...and this is a fairly common issue. Periodontal disease (gun disease) can progress to the point where we get bone loss. I'll save you the big medical words...but basically if the right (wrong?) kind of bacteria are present in gum pockets long enough our own immune system reacts in a way where the jaw bone dissolves and teeth can literally fall out. When it gets to the point, there is frequently infection. What concerns me about this (which in my opinion is a fairly common thing) is that it rarely involves individual teeth. We typically see periodontal disease as a whole mouth condition and weakly anchored teeth (which second and third upper molars tend to me) can be some of the first noticeably effected.
