The diadem could've been tampered with although it's more likely that the reverse legend was recut and then had the camps smoothed (which could explain the loss in weight, although light specimens are recorded).

With the weight issues and comments from siliquae I lean back towards fake. Even smoothed, you would see better detail than on this coin.

 

Follow your gut. I've just had another look at the op image ..... its just not right. Not right at all, mouth wrong, hair wrong, diadem wrong, engraving poor. I'd say fake.

Sometimes standards fluctuated and lighter pieces are to be expected.

As for the discrepancy between obverse head gear and reverse legend, it's easy to see how someone would have re-carved the reverse according to his impression on who the emperor should be from a piece that might've looked like this: https://www.acsearch.info/search.html?id=190536
Just imagine that the lumping dirt and deposits seen obscuring the CONSTANTI part of the legend were instead on the second half. The irregular lettering and the matted fields are the result of dremel tampering.
