Sure they do. From NGC - "NGC will factor white spots into its grading of modern silver coins just as it does for black spots on copper coins and red spots on gold coins. Larger and more distracting spots will result in lower grades. Heavily spotted coins may receive an NGC Details Grade with an Environmental Damage notation." https://www.ngccoin.com/submit/coins-we-grade/white-spots/#:~:text=NGC%20will%20factor%20white%20spots,with%20an%20Environmental%20Damage%20notation. While this quote (and link) is specifically referring to milk spots (white spots), the important part here is what I underlined in the quote. Black spots and red spot count against the grade, and so do spots like the ones on the coin in this thread. Spots of any kind always count. Just like they do in what you find at this link - https://www.ngccoin.com/news/article/7501/learn-grading-plus-and-star/ "To receive a ★, coins must be free of any obvious planchet irregularities, and display no bothersome spots or blemishes." Again, spots always count against the grade.
