Here's a coin that meets the last 2 criteria:

and to contribute to the earlier Lucius Verus run:

On this Trajan drachm of Alexandria both Eirene and Homonoia are named.
    pa0210fp0345.jpg

I would like to see but am unaware of a Provincial with an Olympian god (not a personification) named. So I won't ask for that to avoid another 12 hour break. They are common on Imperials. Why???

Next: An Alexandrian drachm with any legend in addition to or in place of the date. Zumbly's Aelius above is one so we should exclude it here and ask for others. My coin of the type is a hemidrachm so does not count either. I will also exclude the common Nilometer readings (IS) here.

    [START DIVERSION]
A thought-provoking question - while I cannot begin to have an answer, I can add the impression that the legends on provincials seem to be more often about place and recognition of location. Roman approach of adapting to local style/tradition perhaps played a role?

and I will offer a coin that may meet your criteria with the name Athena (Athena Nikephoros - Athena the Bringer of Victory) on the reverse with the bust of Athena on the obverse.
