Usually caused by a grease (or debris) filled die.

There were 5,287,339,926 pennies struck for the 1985-D. How many dies do you think were used to strike all of these coins? Since coins produced from grease-filled dies are relatively common, especially for Lincoln cents, don't you think it would be natural to have it happen fairly often? ~ Chris

Not enough glue to hold the L in place.
    Itt fell off.

Or.. The Mint ran out of the letter L.
