Many times the Mint had the annual sets ready for shipment but the supplier they used to print the COAs was delayed for whatever reason. It was in 1995, when the Mint issued the 10th Anniversary AGE/ASE Proof sets. The set which included the 1995-W Proof ASE arrived just fine - but it came with a note stating the COA would be delivered at a later date.
