5 dollar note with partial missing design

is this an error? If not, what would cause this? Far left top corner near the 5. No missing design on reverse.

A layer of the paper has torn away, damaged after it was made. Think about the security thread. It is between layers of the paper. In this case a layer was torn away somehow.
