2002 D wide,flat,lettering on front and back. what caused this

    Not the best photos, but
    the flattened design/letters
    is from the coin being 'smashed'
    against another object.

    It's not an error, I'm sorry to say.

    Typically, if a coin has a lot of damage and/or discoloration, there is a good chance that anything that looks unusual is also PMD and not an error. This isn't always the case, but something to consider as you gain experience in understanding the minting process and how errors can occur.

    I couldn't get a better one it has this real thick glossy stuff over it and I can hardly see the front and back in person
