In both the 1999 $5 and the 1999 $10, both Washington and Fort Worth began production with tiny runs of 35,200 stars. To my knowledge, no notes have been seen from any of those four runs--neither as circulation issues, nor as part of BEP premium products.

There are a number of other "missing" star runs like this, where the BEP reported printing a run but it was never seen. Not all of them are short runs, either--some are full runs of 3,200,000 notes. For what it's worth, here are the inexplicably missing runs I'm aware of:

$1 1988A I..* run 4 (I09600001*-I12800000*)

$1 1995 E..* run 2 (E03200001*-E03840000*)

$1 1999 H..* run 2 (H03200001*-H03840000*)

$2 1976 B..* run 4 (B01920001*-B02560000*)

$5 1999 BE..* run 1 (BE00000001*-BE00035200*)

$5 1999 BF..* run 1 (BF00000001*-BF00035200*)

$5 1999 BK..* run 1 (BK00000001*-BK03200000*)

$10 1999 BE..* run 1 (BE00000001*-BE00035200*)

$10 1999 BF..* run 1 (BF00000001*-BF00035200*)

$10 1999 BK..* run 4 (BK09600001*-BK12800000*)

$10 2001 CK..* run 1 (CK00000001*-CK03200000*)

$10 2003 DK..* run 1 (DK00000001*-DK00320000*)

The other situation you asked about--where a star run is printed for collector sales rather than for circulation--is less common; I only know of three for sure:

$1 2001 K..* run 2 (K03200001*-K06400000*) - the "Lone Star note"

$10 2003 DA..* run 1 (DA00000001*-DA00416000*) - uncut sheets

$10 2004A GF..* run 1 (GF00000001*-GF00009600*) - uncut sheets

Several other star runs were released in uncut-sheet form too, but in most of these cases, it's unclear whether or not any part of the run was released to circulation directly:

$1 1999 E..* run 2 (E03200001*-E03840000*)

$2 1976 lots of different runs - definitely issued for circulation too

$2 1995 F..* run 1.2 (F00512001*-F00640000*) - definitely issued for circulation too

$5 2003 DG..* run 2 (DG03200001*-DG03520000*)

$10 1995 F..* run 2 (F03200001*-F03520000*)

$10 2004A GB..* run 1 (GB00000001*-GB00640000*)

$10 2004A GL..* run 1 (GL00000001*-GL00128000*)

$20 1996 AL..* run 3 (AL06400001*-AL07040000*)

$20 2004 EA..* run 1 (EA00000001*-EA00320000*)

$20 2004A GA..* run 1 (GA00000001*-GA03840000*)

$50 2004 EG..* run 3 (EG06400001*-EG07040000*)

There, wasn't that more than you wanted to know?
