They say first buy the book, then buy the coin. Same thing here, if you don't know what to look for one might slip by. There is always the outside chance you can find a new unknown doubled die. Some have only one or two or four coins ever found. That tells me there were other times, dates, a few slipped by quality control for a few minutes but 5, or 10, or 1000 coins out of say 5 1/2 billion coins for a particular mint in a particular year is going to be DARN hard to find.
