1969 D Jefferson Nickle Error?

    Hi Folks,

    I came across the pictured 69d Jefferson and it really stood out to me. I can't find any reference to an error of this type. In your opinion, is this an error strike or some odd happenstance?
    The double strike on the obverse is clear, there is not a corresponding strike on the reverse, but it is very weak through the middle.

    Note how the word STATES is reversed on the obverse of the coin. This tells us that two coins were smashed together.
