Welcome to CoinTalk. Another Hoosier here. ;) I don't see any errors on the coin you posted. Looks pretty normal to me.
The filled D is due to a broken die post. Common on these cents. The T and 1 being flattened is due to circulation wear and tear...not a mint error.
A bit late to this thread, but yes, it was in one of those "lucky penny" holders. One way I look at coins I think might exhibit a mint error is...
