The first one is PMD because you can see that the reeding was fully intact before the damage occurred. The second one appears to be polished...

I think it's just a reflection on the left sides of the letters. It's machine doubling. Even if it was DDD, it's still only worth 10 cents.

65 or 66. Definitely had some clash marks polished out on the reverse. Cool find.

I have no idea how it happened, but it is PMD. It is fully struck on both sides with no damage, so we know it was flat when it left the mint.
It's not worth resubmitting IMO. Just sloppy quality control at the mint which is normal these days.
They gave it a 69 with that on it?

Actually, the ERT look shifted up, so it might just be a PMD gouge which has had 60+ years to blend in with the color of the coin.
It does look like it, but I can't say for sure. Still, that's a very cool coin.
It could be a partial collar strike. Is the reeded edge normal all the way around? If it is, then it's within mint specs and nothing special.
