Try google for die cracks. You'll see photos of the crack as raised. Very easy to spot.

Foam is not good for coins.
All the letters were the same at the Mint until the Die wore down, then add in circulation and you have this coin.
Hard to say but it looks like a hunk of metal slag from a smelter.
