A worn die, an over polished Die or a grease or debris filled Die would cause this. They all fall under the term of no added value.
This is a great example of a worn die. A definite keeper.

You just wiggle the end keeping it in tact and in goes the smaller dime. In a few minutes you can have what you see.

They put up a very expensive satellite so for the last few years you have the data available. A manager can tell the exact location of the scan....
Call them or break it out and send to another TPG.
