looks like a photocopy. How about a clearer picture, or even a lighter scan.
The missing 'US' is most likely from a greased die. the doubling, machine doubling (aka: strike doubling).

It is a PCI slab, so it can't be that bad. The Gold PCI's were done to improve their image, and I believe that they were getting more conservative.
