They usually don't tell you. The US Mint switched over in the past few years, but I can't tell you the exact date.
Depends on the dealer, they all use different percentages. Many use 10%, meaning 10% less than whatever the spot price is at the time.

And I would say that 75-80% of professional graders would call it a 66. Remember tex, when a TPG grades a coin, it's not just 1 person doing it....

The '58 Mint Sets are famous for producing some of our most spectacularly toned coins. Can't tell you why they did, just that they did.
