It looks like a well struck coin to me. There was probably some grease or other material in the VDB when it was struck. Not all that unusual...

Most likely grease got in the die. Not all that rare and not recognized as anything special.

It is very common for that to be weak particularly in early coins. That point is opposite the shoulder and there is only so much metal to go around.
