Spacing is off, if the left mark is the original location, then it is too far from the 4.

Damage doesn't increase the value. It is just worth a cent, but in that condition it may not even be accepted.
It looks circulated and harshly cleaned to me. Worth only a few dollars and no way it would straight grade.

That is just the Denver mintmark. Value would depend on condition but it is a very common coin.
Yes it was altered from a regular quarter, part of a magic trick folding quarter.
It is one part of a "magic folding quarter" It consists of two pieces with a rubber band in the groove around the coin.
The only valuable 1982's are the 1982 D small date copper, to date only 2 have been found.

It is just a Philadelphia minted coin, they did not carry mintmarks until later. Somewhere in the 80's I believe they started using a P mintmark.
