It's a sure thing that this is post mint damage. We can only guess what tools were used to create it.
Why does this happen with only some dies? Were the dies not treated/hardened properly or was the press not setup correctly? Too much pressure or...

I don't see a way to tell from the images but I'll trust PCGS is close.
