Not an error but plain and simple machine doubling.
Even with the poor blurry photo I can see that it is clearly damaged.

It's damaged. Spend it before it rots into oblivion.
It's a weak spot on the coin. Add in a worn die and split plate doubling starts occurring. That's what you have.
