It is a genuine coin, but is ungradable due to the fact that it has been cleaned.

I think it is a mix of mechanical doubling and offset die deterioration doubling.

The coin looks very circulated, so I would attribute any extra thickness you see to circulation wear and die deterioration.
Could be a combo of both.

A die crack would be a crack in the die... this is a die chip. A piece of the die fell out. It was a crack at one point but developed into a chip.
