I believe it was the 1992 D they used Proof dies on the reverse. Not this one.
With the second 5 doubled I will go with die deterioration doubling. As you said about the blur, too hard to make any other comments on doubling.
If it is flush and does not have ridges, it is likely a minor struck through, grease or other debris.
