It’s one of PCGS’s earlier slabs. More of a promotional slab. Sample slabs have SAMPLE on the label. You should have bought all of them at that price.
As they said, probably damage but we need photos to verify. We’ve seen this before. Copper is soft and one hit can move the metal.
