Acid will dissolve the pure copper core faster than the copper/nickel alloyed outer layers. Your quarter was subjected to an acidic environment....

A coin rolling machine counts and then rolls coins in a wrapper. Spinning wheels push the coins through the machine as well as crimping the ends...
A bump of some kind flattened the last 9. It didn't come out of the dies like that, it's damage.
