I would call it Fine by American standards. British standards, maybe VG? I'm not so good with y'all's scale.

That is a proof, so it is definitely a type II. Remember, most proofs in 1956, and all proofs after that, had the Type II eagle.
My best guess to produce an effect like that would be a double struck coin, second strike slightly shifted. If that is the case, great find!
