I don't think it is your focus. Try playing with the lighting. You might also try backing your camera up a bit. Your pictures are way larger...
You might be right, but I think it would go AU. But with all that glare where I am trying to see, that would be far from assured.

Could it be an end of run strike? i.e. the press was stopped before it had a full strike

Well, you are in the right territory. However, you pictures are so fuzzy, that (and that you are probably not overgrading) is about all I can say.

I am pretty sure that all proofs have the dot centered between the D and B. Yours does not. Sorry, but I doubt it is a proof.
Basically, yes. The defect in this case is the cleaning.
They will not grade it. They will call it genuine, VF details.
Now I am as sure as one can be from a picture it has been cleaned.
