I think it was caused by a worn die. There are ridge rings on both sides as well, which are more evidence of die deterioration.

I don't have a firm stance on planchet flaws yet. Grading is still new to me, I only have two and a half years of experience.
There are a few marks on his cheek, but I think it deserves MS-65.

I believe there's also one that was clashed with a obverse Double Eagle die.
