Put the tube in warm water, and then try to get them out. This will expand the tube. If that doesn't work, just try breaking open the tube.

Yeah, it's most likely someone doing it because they are bored or just felt like it. Also, IMO, the first coin is missing partial clad layer.

If it is the same size as a quarter then it is a quarter. Its a capped bust something.
It looks like a damaged die and a MAD, but I don't know much more than that. It seems too damaged for the employees not to notice. @JCro57
The 1909 penny is just damage. Other ones aren't bad though.

Surprised it's MS, with all the fingerprints and marks on it. But I can agree with cleaned on the label. You can see the discoloring all around...
