That is just the Denver mintmark. Value would depend on condition but it is a very common coin.
Yes it was altered from a regular quarter, part of a magic trick folding quarter.

It is just a Philadelphia minted coin, they did not carry mintmarks until later. Somewhere in the 80's I believe they started using a P mintmark.
