I have noticed that PCGS often assigns "plus" grades to coins (let's say Morgan Dollars, for example) that display prooflike/deep-mirror prooflike...
Obverse - MS65 Reverse - MS66 PCGS probably called it MS65 due to the chatter on Washington's cheek, but I could see it with a "plus" grade... :)

Yes - it looks like a minor die crack or gouge. Doesn't really carry any premium, but it's still neat to look at. :)

Diagnostics don't match-up with any die pairings for the "No 'D' Weak/Strong reverse" varieties. I concur - just a "Weak 'D'"
