It looks almost like the metal was pushed/moved. I see this a lot on copper Lincoln cents but I've not seen it on a silver coin before.

Never seen foil lined bubble wrap used with before. Where did it come from? The only thing I could think of is that they didn't want it to be seen...
It's uniface because the reverse was ground off as is evidenced by all of those scratch marks.
Definitely from a teller stamp. EDIT: Think about where the strap goes...right around the middle. Then they stamp the strap and sometimes it goes...
