Not sure of the cause on the coin but the italics in your story are annoying. I'm sure of that.
The doubling you see on this is from die deterioration. Basically the mint used the die more than it should have. Not an error for collecting.

Gaining dimes in the cent rolls happens. Has happened to me a few times.
