What would cause this?

    ground down, some worker bored at lunchtime...

    Post minting damage (PMD) is correct, the coin was sanded/ground down.

    It does look like someone was trying to make a button to me.

    Sort of like mounting a loop, just more extreme. Is the coin the OP shows a ground down cent or a cast using a cent as the mother?
