If I'm indeed looking at this correctly, the cause was likely an over-polished die.

 

When I first saw it I thought that too, but it is incuse or deeper than the fields. There is also a distinct incuse edge below the eye on the cheek.

 

There is another spot over the ear.

 

Looks like Washington has a band aid on his nose.
