I was just wondering with all the heavy minting methods (sledge hammer, steam press, etc.) has there ever been an accident which claimed a human life?

I can't find anything about workplace deaths at the mint but judging from this actual photo of the original Philadelphia Mint,it's surprising the place didn't collapse.Of course this photo was obviously taken over a hundred years after it was built as evidenced by the car parked out front...or is that a hearse? :)
