Strike through, Lamination, kid having fun?

I want to know if this 2013 P nickel is an error or if a child just had a little fun with some power tools.

This doesn't seem to be a lamination.
I am thinking an extremely worn die and possible struck through debri.

 

It looks like there might have been extensive grease and debris caked on the face of the die.

NOTE: It would help if you would take your photos from directly overhead and place your light sources at a 45 degree angle to the subject so the light doesn't bounce back into the lens. Before you post the image, you need to crop all of the extraneous background and then resize the image.
