In 1970 a worker at the San Francisco mint put ONE 1941 Canadian quarter in the press and over struck it with proof Washington quarter dies. It was nothing official, just one worker creating a rarity for himself. (He or another worker also struck a 1970-S quarter over a 1900 Barber quarter.)

Big Kev should stick to retailing weed on the corner, but stop using his own product.

The single assisted error is true, it's been authenticated by a TPG and is for sale by error dealer Mike Byers: http://www.mikebyers.com/1949701-013.html
