railroad rim? pmd? advice appreciated

i read something about a railroad wheel rim? is that a real thing? does it look like this? if not, does anyone have a an example?
what is REALLY going on here? above IN there looks like a ghostly image of it? it looks like the I in liberty is pushed over.
